The Core Plan includes the following on-site medical services during its validity period:
- 12 Primary Care consultations or follow-ups with family practitioners / GPs, internists in person or via telemedicine. The standard visit is 30 minutes; patients requiring more time will use two visits.
- 10 visits of any of the following FirstMed medical services:
- Any listed FirstMed Specialist
- Radiologist (Ultrasound)
- Annual Gynecological Exam
- Vision check
- Travel consultation [vaccines will be extra]
- Dietitian consultation [60 min– takes 2 visits]
- Physical Therapy [30 min]
- Mental Health [Psychology or Psychiatry [60 min – takes 2 visits]
- On-site imaging diagnostics, if medically indicated: x-ray/mammography
- One occasion per plan year of each of the services:
- Annual Health Maintenance Exam with follow-up (HME)
- Flu shot (3 component)
- Dental check-up (Fürstner Dentistry)
- 10 online prescription refill occasions.
- FirstMed basic labs – when ordered by a FirstMed doctor
- COVID tests: When you need a COVID antigen test to see a FirstMed Doctor. Requested or further tests are with a 35% discount from the full price list.
- Anything beyond the benefits covered in the plan will be covered with the Value Card pricing model or with a 35% discount from our full price list.
Exclusions and Additional Charges
The Core Plan will not cover any medical services not specifically mentioned above. FirstMed specialists are limited to those currently contracted. The exclusions are not limited to the list below:
- Specialist Procedures: The plan does not cover specialist procedures, including but not limited to: Cardiology, Neurology, Minor Surgery, Gastroenterology, Pulmonology, Sleep Medicine, Allergology, and Endoscopy. Procedures will be priced with the Value card or a 35% discount, whichever price is less.
- Long Specialist Visits: Visits requiring 60 minutes of the doctor’s time will count as two (2) visits except the Annual Health Exam. First-time Neurology/Diabetes / Pulmonology / etc. visits are all 60 minutes.
- Drug Abuse, Alcoholism, and Sexually Transmitted Diseases: Diagnosis and treatment of conditions arising from drug abuse, alcoholism, or sexually transmitted diseases are excluded.
- Laboratory Tests: The plan covers basic lab tests ordered by your FirstMed doctor. Patients requesting their own labs will be asked to pay at the time of service. Some specialized labs are not covered by this plan. Please ask our receptionists about the laboratory tests included in the plan. The plan holder shall pay for all other lab tests with a 35% discount from the full listed price.
- After-Hours Visits: After-hours visits require an additional charge.
Core Plan Terms and Conditions
Please note the following below:
- Specialist Availability: FirstMed covers medical specialties currently available at our clinic. Specialist availability is subject to their schedules and employment status. We are not required to provide a replacement if a specialist is unavailable due to full schedules, holidays, or departure.
- Additional Services: Additional services or consultations beyond those included in the Plan can be obtained at discounted prices offered to Value Card holders.
- Annual Health Maintenance Exam: ONE HME per year. If the planholder takes more than one exam per year, they will be charged the ‘Value Card’ price for the additional exam.
- Consultation Appointments: An appointment covers a routine exam time slot up to given times: primary care (30 min) or specialist (20/30 min). Exams requiring more time will count as two visits. [ex. HME]
- Missed or Canceled Appointments: Missed or canceled appointments within 24 hours count towards the visits included in the plan.
- Follow-up Visits: ‘Follow-up’ visits count towards the plan totals (except HME).
- Additional Costs: Services provided or arranged by FirstMed that are not included in the ‘Core’ plan may incur additional costs. Please inquire about prices prior to treatment.
- Outside Appointments: FirstMed is not responsible for organizing non-FirstMed. Planholders will be provided contact information for external consultants or facilities but must book and pay at the point of service.
- Plan validity: The plan is valid for 1 year from the date of purchase.
- Loyalty Discount: To maintain the loyalty discount, plans must be renewed promptly upon expiration and paid within 7 days of the expiration date.
- Group Pricing: Group pricing is available only for group plans with the same ‘Start’ and ‘Expiration’ dates.
- Transition to Group Plans: If you purchase an individual Plan and wish to convert it to a group plan, additional members can join within seven (7) calendar days. The expiration date for subsequent planholders will align with the original plan’s expiration date.
- Group and loyalty discounts may not be combined.
- Anything not specifically covered by Section 1 is excluded.
