Choose the Elite Plan – Adult for comprehensive medical attention over a year, as it includes the following services:
Comprehensive medical care
Unlimited primary and specialty care visits
- Unlimited primary care consultations or follow-ups with family practitioners / GPs and internists in person or via Telemedicine.
- Unlimited in-house specialist consultations or follow-ups, if medically indicated, with listed FirstMed specialists not limited to the following:
- Allergy Specialist
- Cardiologist
- Dermatologist
- ENT Specialist
- Gynecologists
- Neurologist
- Ophthalmologist
- Orthopedist /Traumatologist
- Radiologist
- Surgeon
- Urologist
- View the full list of medical specialties.
- Unlimited prescription refills.
- Medical exams for permits and licenses: work and residence permit, driving license examination.
- Travel consultation (not including vaccinations)
An expanded number of additional medical services
- 10 visits per year of each of the services below
- Minor surgical treatments: mole, birthmark, and wart removal, sclerotherapy, lipoma, abscess, furunculous, folliculitis, hemorrhoid–thrombectomy, anoscopy.
- Mental health consultations (psychiatrist or psychologist sessions)
- Physical therapy treatment
- Nutritionist consultation
- 1 per year of each of the services below
- Flu shot (3 component)
- Dental check-up (Fürstner Dentistry)
- After-hours urgency care (at an additional charge).
Annual screenings
One occasion of the following preventive exams:
- Comprehensive health maintenance exam,
- Women’s health exam (including Pap smear, vaginal culture, physical breast exam, mammography)
- School and sport physical exam
On-site diagnostics
- Imaging diagnostics, if medically indicated: x-ray, single region ultrasound (3x), mammography*, and
- FirstMed basic lab tests**.
- COVID tests: When you need a COVID antigen test to see a FirstMed Doctor. Requested or further tests are with a 35% discount from the full price list.
Exclusions and Additional Charges
The Elite Plan will not cover any medical services not specifically mentioned above. FirstMed specialists are limited to those currently contracted. The exclusions are not limited to the list below:
- Outside consultations / Home visits / Hospital care / Ambulance transportation
- ENG / EMG / EEG diagnostics / CT scan / MRI diagnostics
- ECHO / Stress tests / Sleep Study
- FirstMed Endoscopy Center procedures
- Prenatal care (included in our Prenatal Plan) / Infertility / assisted fertilization treatments)
- Orthopedic procedures and orthopedic medical supply rental
- Medications, vaccinations, disposable medical supplies (i.e., bandages, casts, splint application, etc.)
- Dental care
- Holter/Blood pressure monitor for more than 24 hrs.
- Immunotherapy
- Diagnosis and treatment of conditions arising from drug abuse, alcoholism, or STD/STI.
- Routine medical maintenance or treatment of chronic conditions, e.g., frequent injections or diagnostics, may be accepted on a case-by-case basis but otherwise are excluded.
- *With breast implants, FirstMed is unable to provide mammography. An outside screening is not covered by the Plan.
- **The Plan covers basic lab tests ordered by a FirstMed doctor. Please ask our receptionists about the laboratory tests included in the plan. All other lab tests, as well as ‘patient requested’ ones, shall be paid for by the plan holder.
Elite Plan Terms and Conditions
Please note the following terms and conditions below:
- Specialist availability: FirstMed covers medical specialties currently available at our clinic. Specialist availability is subject to their schedules and employment status. We are not required to provide a replacement if a specialist is unavailable due to full schedules, holidays, or departure.
- Specialist procedures: The plan does not cover specialist procedures, including but not limited to: Cardiology, Neurology, Gastroenterology, Pulmonology, Sleep Medicine, Allergology, and Endoscopy. Procedures will be priced at the Value Card price or a 35% discount, whichever is less.
- Additional services: Additional services or consultations beyond those included in the Plan can be obtained at discounted prices offered by the Value Card terms.
- Consultation appointments: A consultation appointment covers a routine exam time slot up to given times: Primary Care (30 minutes), Specialist (20-30 minutes). Examinations requiring longer than this period shall be counted as two visits.
- Missed or canceled appointments: Missed or canceled appointments within 24 hours count towards the visits included in the Plan. Repeated ‘No-shows’ or late cancellations may be grounds to terminate the Plan.
- Follow-up visits: ‘Follow-up’ visits count towards the plan totals (except for Health Maintenance Exam).
- Additional costs: Services provided or arranged by FirstMed that are not included in the scope of the Plan may involve additional costs. Please inquire about prices prior to treatment.
- Outside appointments: FirstMed is not responsible for organizing outside appointments. Contact details for external consultants or facilities will be provided, but planholders must book and pay at the point of service.
- Plan validity: The Plan is valid for one year from the date of purchase.
